Appointments

My preferred method of contact initially is either by e-mail or phone.  I try to answer all calls if I am available.  Chances are, if I do not answer my phone, I am giving someone the same attention and respect I will give you when we are together, so please leave me a message and I will return your call at my earliest convenience.

I have included an appointment request form below.  If you are uncomfortable filling it out and prefer to send me an e-mail instead, please be sure to include the same information that the form asks for.

References are required: I need the name of a well reviewed INDEPENDENT Provider that will vouch for you along with her e-mail address and phone number…. NO EXCEPTIONS.  I do not accept agencies or ladies that work for agencies as a Provider reference.  If it has been a while since you saw your provider reference it would help if you include when and where you saw her in case she asks for that information.
